  1. What if I change my mind and decide I don't wish to receive my tax form electronically, but I've already given Consent?
    You may withdraw your Consent to receive your tax form(s) electronically at any time. However, Consent for Electronic Delivery cannot be withdrawn on ViewMyForms.com. You must notify the Company/Institution issuing your tax form(s) in writing, either electronically or on paper, that you wish to withdraw your Consent.
  2. After giving consent to receive my form electronically, what if I decide later that I want a paper form mailed to me?
    Please call the phone number listed at the top of your tax form to request a reprint of your form be mailed to you. You’ll find the contact phone number in the upper left box on your tax form under the name of the Company/Institution that issued your form.
  3. How long will I have access to my form on Viewmyforms.com?
    Per IRS regulations, your form will be available to you through October 15th of the year directly following the Tax Year shown on your form.
  4. Now that I’ve given Consent to receive my form electronically, does this mean my tax form will be delivered this way every year from now on?
    That is correct. You have given Consent to receive your form electronically for the current Tax Year and for all future tax years.
  5. How do I request an edit to the information shown on my tax form?
    Edits to your tax form cannot be made on Viewmyforms.com. You must contact the Company/Institution that issued your tax form. Please call the phone number listed at the top of your tax form to request any revisions and have a revised copy of your form mailed to you. You’ll find the contact phone number in the upper left box on your tax form under the name of the Company/Institution that issued your form.
